Nicolas Jackson then scored a header shortly afterwards after some good work from another substitute, Reece James, to give Chelsea all three points.
Chris Waddle impressed with Chelsea star before winner
James was making his first appearance since December after recovering from a hamstring operation and did not disappoint.
It was his ball into the box that Jackson put away, and former Newcastle United winger Chris Waddle praised the right-back for his delivery.
Commentating on the game for the BBC, Waddle said: “It is a great ball in by Reece James and nobody tracks him. Nicolas Jackson three-yards out powers it into the net!
“Let’s be honest Chelsea have not been great and there has been nothing between the two teams.”
Following their win at the City Ground, there is now a real possibility of Chelsea finishing in the top six this season. The Blues are now level on points with Newcastle, who currently sit in sixth.
Should Reece James start against Brighton & Hove Albion?
Next up for Chelsea is a trip to the Amex Stadium to take on Brighton & Hove Albion.
James has just returned from injury, but if he is fit enough to start, it is something that Pochettino should really give serious consideration to.
The Chelsea captain, who earns £250,000 a week (Spotrac) at Stamford Bridge, has once again shown the kind of threat he brings to the team going forward.
Of course, with Malo Gusto also fit again, there is no need for Pochettino to risk James. But again, if a weapon like the England international is available, it may be wise to utilise him.
